Raise an ethics concern
Use this route to raise a concern about conduct in IAHPM activities or the unauthorised use of the Association's name. Describe the relevant events and provide information that helps identify the matter.
What this route covers
Relevant matters can include conduct in Association work, undisclosed interests, misuse of restricted Association information and misleading claims about an IAHPM relationship. The applicable procedure determines how a concern is assessed and handled.
Other matters
For an account problem or ordinary membership question, use member support. Product faults, suspected safety incidents and individual healthcare concerns should be directed to the manufacturer, appropriate authority or relevant care service as applicable. This form is not a product-vigilance or emergency service.
Information to provide
Describe what happened, when it occurred, the Association activity involved and the basis for your concern. Separate what you observed from information received from others. Avoid including unnecessary personal or health information.
If you request confidential handling, explain the concern. The handling arrangements and any limits to confidentiality are assessed under the applicable procedure; absolute secrecy cannot be promised through this form.
Handling the concern
The designated recipient assesses whether the matter falls within the procedure and whether further information or referral is needed. A person whose involvement creates a conflict should not determine the handling of that concern.
The procedure has four stages: record the concern; assess scope and conflicts; seek proportionate clarification; record the outcome and any action. Access is limited to authorised handling personnel. The secure follow-up record allows the reporting person to review a response and provide further information.
If the concern involves a person who would ordinarily handle it, identify that conflict. The person concerned must not decide the outcome. Where an independent assessment is needed, the handling arrangements must be agreed before the substantive review. This channel is not a claim of statutory whistleblower protection and does not replace a report to a competent authority.
A report should be made in good faith. No adverse treatment is authorised solely because someone raises an honest concern. The process does not prevent a person from seeking independent advice or using an applicable legal reporting route.
