Use this form to introduce your organisation and request membership review. Please read the eligibility criteria and membership responsibilities before submitting an application.
Before you begin
You will need the organisation's legal name, registration jurisdiction, website, a description of its activities and an authorised contact. Select the category that best reflects the organisation's role. The membership team will confirm the category during review.
Do not include confidential technical files, personal health information or payment-card details in this form. If supporting documents are needed, the team will explain how to provide them.

What happens next
Once the application has been received, the membership team reviews the information and contacts the authorised representative about any further requirements. The membership decision, applicable fee and effective date are communicated separately.
